• Patho physiology of Osteoarthritis? I. Primary osteoarthritis is mostly related to ageing smooth cartilage at the bony end wears down (degenerates) leading to tiny cracks in the cartilage. II. This cracks gradually increases and there is the total loss of cartilage cushion between the bony ends of the joints. III. This increases the friction and so there is pain. Sometimes small flakes of cartilage and bone are set loose in the joints, these are known as ‘loose bodies’. IV. Bony protrusions are seen from the side of the bony ends, these are known as ‘Osteophytes’. V. Small cavities occur below these cracks, these cavities are known as ‘Subchondral cysts’. VI. With increase in pressure of weight bearing there is increased density of bone which is called as ‘Sclerosis’. VII. Due to sclerosis there is congestion of blood at the bone ends leading to raised Intra vascular pressure, this leads to pain.
